We are seeking an experienced Advanced Practice Provider (NP or PA) to join our outpatient Endocrinology team with a focus on diabetes management. This role is dedicated to providing comprehensive care for patients with Type 1, Type 2, and complex diabetes conditions, supporting optimal glycemic control and overall health.

Key Responsibilities:

• Perform detailed assessments and develop individualized diabetes care plans.
• Initiate and adjust insulin therapy, oral medications, and advanced diabetes technologies (CGM, insulin pumps).
• Provide patient education on nutrition, lifestyle, and self-management strategies.
• Collaborate with endocrinologists, dietitians, and diabetes educators to ensure coordinated care.
• Monitor and manage acute and chronic complications related to diabetes.

Qualifications:

• Experience in diabetes management or endocrinology strongly preferred.
• Familiarity with insulin pumps, CGM devices, and diabetes education best practices.

MINIMUM QUALIFICATION(S):

• Graduate of an AMA approved Physician Assistant Program or baccalaureate or higher degree program that meets licensure requirements in the State of employment
• National Certification in State of employment or in another State with reciprocity and licensure to practice in the State of employment
• Basic Life Support (BLS) certification

PREFERRED QUALIFICATION(S):

• Experience in Endocrinology
