Nurse Practitioner/Physician Assistant (Home Base PC)
The Home-Based Primary Care Physician Assistant or Nurse Practitioner supply medical care of patients, identifying medical problems, defining the medical management, prescribing medications and treatment in accordance with their individual scope of practice in the patient's home. Also functions as the HBPC provider for patients and coordinates care among all members of the health care team, including staff, outpatient primary and specialty physicians, inpatient and ancillary hospital services.
