Job DutiesThe SNF Advanced Practitioner will work in conjunction with General Internal Medicine clinical staff, applicable insurance case management staff, and skilled nursing facilities to ensure patients are appropriately managed in the sub-acute and/or long-term care setting. Also, provider provides direct (hands-on) care in nursing homes/skilled facilities with a goal to improve quality and prevent unnecessary re-admissions to the acute care setting. EDUCATION AND EXPERIENCE An experienced or newly graduated, with nursing home experience, Nurse Practitioner or Physician Assistant. - Must have certification of completion from an approved program for Nurse Practitioners/Physician Assistant and have or be eligible for current license in the Commonwealth of Pennsylvania. - Must have BLS and have or be eligible for ACLS. - Prior experience in nursing home/skilled nursing facility billing (codes, understanding of medical necessity). - Prior advanced practitioner experience in a long-term care setting and/or administrative experience in a skilled nursing facility or long-term care facility is preferred.
